Extremely Weird And Obscure Games

Garage: Bad Dream Adventure pretty much ticks every box of the criteria you listed. Extremely obscure Japanese PC game from the late 90's.

Mouthwash, seriously, it gets creepy at times, not because it having cheap jumpscares, but because it feels too clausptrophobic, instead of an invading force, the game deals with space isolation and is a effective take on realistic horror despite the sci-fi setting
 
I would recommend Hellnight and Drowned God, two of the weirdest games I've ever played. The former is a horror dungeon crawler where you explore an old web of tunnels underneath Tokyo, called Tokyo Mesh while being hunted down by a monster. It's a truly bizarre and quite unique game.

The other, Drowned God, is a point and click adventure game. Touches on a lot of esoteric and occult topics, alongside many conspiracy theories. It's quite a special case for me as it feels like the author, Harry Horse, genuinely believed in some of these theories and ideas, which makes the execution much more unique than any other game that tried to be esoteric or similar for me.

Maybe you've heard of it already idk, but it's the one where you're at this amusement park of sorts & you play the game in horror movie episodes.
Like I played it myself & the game has issues for sure but the game is so off the wall (basically at all times) that yeah, it's a one-of-a-kind experience, you will not see this anywhere else for sure!
Gameplay is largely a labyrinth navigation game with (repetitive) search-&-find elements in short.

The only game where you play as Woody fighting evil sonic.

A 2000s psuedo first person Adventure/RPG by Atlus for the Sega Dreamcast, where the player takes on the role of a psychic soldier exploring a post apocalyptic Japan. Your party members are these bizarre parasitic organisms you employ to partake in psychic battles with messed up cyborgs and mutants.

There's no complete or official translation of the game itself. But there's a guy on Youtube who "subbed" the game using CC.

I haven't played much horror games but I would recommend Theresia: Dear Emile, Nanashi no Game, Dementium II and Corpse Party.

Well, right now I'm playing Super Galdelic Hour for PS2. It's certainly conceptually bizarre and something that would never appear on PlayStation today. Fanservice aside, I like the weirdness, the funky take on futurism (like Space Channel 5 in some ways), and how the graphics look impressive for PS2 standards:

When I want surreal or bizarre games, I learn about them mainly from Alpha Beta Gamer. For something along the lines of "this game shouldn't exist", there's Kotel ne Gori:

Thank you ever so much for the great suggestions here. The Super Galdelic reminds me of DOA extreme volleyball I think it was. Where you had all the weird beach games that was basically fan service! It does look fun however.

I have heard of some of these but because they're indie PC games, the limits are alot looser. Ideally I'm looking for anything PS2 or before but I am open to them.

And growing my grandpa.....what the hell is that?!?!

Both ps1 and saturn has english patch
I think ps1 version is better since it come out after saturn and had some bug fixes

Wii/PS2 3D version. Third person viewpoint for better visibility, native English voice and dialogue, pretty slick anime opening, and it's a lot smoother and cleaner to play than the PS1/Saturn version.

However, the grunge on the OG sprite versions led to a lot of the game's charm, since, it felt like you were walking around a grim painting rather than looking like pretty much every mid game from the 2000s 6th/7th gen consoles. It's creepier too.
