i heard this game was kind of like 'extreme' horror or something... but i looked at some clips and it just seemed like a standard point and click in a fantasy mental institution. doing busy work puzzles like in monkey island seemed to detract from the atmosphere

It is, but the plot is just bonkers.

this tbh, the game is more of a classic for its plot and genre rather than anything else. perhaps younger folks would call it a hidden gem...
 
My personal Japan-only White Whale game that with each passing day tempts me to learn Japanese is Sakura Note. It's directed by Akira Ueda, who's most notable for directing and being the art designer for Contact, and just like Contact, Sakura Note's pixel art is absolutely beautiful. It's also composed by Nobuo Uematsu of Final Fantasy fame and has a great OST. It seems to be an adventure game about growing up with bizarre supernatural stuff happening and where you have to fight a giant pig at the end of the first chapter. I desperately want to know what the writing is like, since I feel like there's a good chance this would wind up being an all-time favorite for me.

Tried to pick the ones i think are the best along with not being well-know or forgotten in time. I was debating if i should include God Hand here, but i feel like over the years it has lost it's "hidden" status. At least i've already seen lots of coverage about it.

God Hand would've been completely under the radar if not for a couple of reviewers back in the day that made a big deal out of it. For those who didn't buy the magazines it would've been a huge hidden gem.

Good call on Genji - it wasn't a common game to find on stores shelves (at least 90's/20's Florida). Think I played through the first half of it back around 2005) definitely something to, at least, give a try if your new to PS2.

Gitaroo Man was made popular by word-of-mouth in 20's forums, otherwise that too would've went undiscovered.

Are they playable for a non-Japanese speaker?
 
Not sure if it's obscure or hidden gem, but among the titles I've played on ps2 I think "Stella Deus" it's the less AAA (I played FF10 and 10-2, Odin Sphere, DQ8, DOA, Soul Calibur, YS Napishtim, Atelier Iris 3 and Grandia 2, also Shadow Heart, Dawn of Mana and Dirge of Cerberus on emu).
