You got it! It's just called Mega Man, the only such game on the Game Gear. It has levels from 4 and 5, so I can see how someone would be mistaken. The only real hint was it being screen-crunched as Ranmy said.

It was originally released for DOS and has a Windows version too. It was never in arcades to my knowledge.

To my knowledge, it was commercially released in Taiwan. It also has a MUGEN remake.
