I used Neko Project ii. I just booted it to dos. PC98, if you look into the name, is a PC, the first two letters give it away. PC98 uses some same stuff like x86 compatible processors (at first, likely NEC's own clone, I seen one myself in a Zenith once) and other NEC's own hardware, but it is PC, it has x86 CPU, and it has MS-DOS licensed. You do not need DosBox to run DOS, you just need DOS, but DOSbox like the name implies includes a likely open source reimplementation of MS-DOS. Older machines especially used to boot MS-DOS from a diskette and then when it is running you just swap discs you need to use as the OS resides in memory. Like said, I tried to figure out how to make Neko Project ii kai look like a FD+HD instead of 2FD but I could not get to that, I could not get DOS 6 installer or the game's own setups detect any HDD even if I tried to mount IDE1 and there someone who knows this stuff better or a good tutorial is needed, which was already a lot of work as is figuring out how to setup a smut game on Japanese computer platform entirely in Japanese. Like said, if you find one of those NEC MS-DOS disk images that is a booter, I found one for 5.0a-h by NEC for PC-9800 specifically.
TL;DR: You just can boot DOS on a PC98 and thus NekoProject, you just need DOS. I have to see if I can help you any more but like said I am unaware how to setup NekoProject and the language barrier is bit high trying to figure out how stuff works. Very unique, especially user discs that is very common in Japanese PC games throughout. I first managed to format the actual game images with it's setup which was fun. If you look at the box scan too, it says MS-DOS 2.11 Required (?) and likely something about the 2FD or FD+HD operation.
I also give up hard since I cannot make heads and tails what the emulator is doing. sometimes disk images I have not even inserted show up in B drive, sometimes FDD1 shows up as B sometimes FDD2 and so much more confusion in trying to figure out a usable PC thing with NekoKai.
This game is not easy to setup, if you are not willing to figure out or find a ready made HDD image with MS-DOS and everything then booting the game from the RIGHT A: drive path (for me, for whatever reason, the mounted HDD became A: drive so that broke everything I could attempt) - This game is very particular in how it is run and setup and it leans on the MS-DOS. It is not easy, I give up after I cannot make Neko Project itself function in any logical manner before there can be even an attempt, for some obscure smut game.
