The main one I always remember was someone on the staff was carrying a tray of food to the Bain-Marie, the big thing like this that keeps food warm:

They dropped the food on the carpeted floor, scooped it back into the tray and placed it in the Bain-Marie anyway.
 
I did a food safety course like a decade ago, and people there had plenty of horror stories about a local all-you-can-eat Chinese food place. Safe to say I never ate there again...::surprised-pikachu
The quickest way to never wanting to eat fast food again is asking a fast food employee about there working environment

My friend's brother used to be the manager of a local McDonald's, and I was told a few times to never eat there. The restaurant was clean, but you know when they have specials, like buy one get one free and the like?

That's often McDonald's HQ making them get rid of old stock that has just reached its printed expiration date. It's still "legal" to sell, but questionably safe. I was also told that if any of the burger patties or chicken patties had any mold on them, they were to be cut off, and the rest of the patty would be used for snack wraps. I've known the guy for as long as he's been friends with my brother.

He's pretty honest, so let's just say I haven't eaten at McDonald's since.

Heard the same stories from other people about the same thing

I only go to McDonald's for the occasional happy meal toy that I buy on its own. I might be better off trying to eat those than the actual food::surprised-pikachu
