Well, my First Puzzle game is possibly this one:

Although he is considerate an unlicensed game but it still counts because it's a puzzle game in itself.

I don't really know, but whatever it was, it was also my first game. I only remember it was a point and click game for windows XP and a small section. I was in some kind of tower and I was trying to find parts for a steampunk flying machine.
I doubt I'll ever remember what it was because I was 3 and I played it once.

I feel like pre-Tetris, I had something on the commodore that was like a sliding tile puzzle, but I can't quite place it atm.
 
A draw game between Magic Jewelry and Tengen's Tetris, i loved playing the 2D mode of the later with my big sis... she always beat me, but it was fun
 
I played Dr. Mario in both the GB, NES and SNES versions.
I haven't played SNES but I did get the NES version a few years back. I have fond memories of Dr Mario on GB as I was given it and the Gameboy as a gift when I was 8-9 because I was in the hospital for a few weeks lol

Both puzzle games are fun although Tengen's Tetris is more fun because it can be played as a 2-player in competitive or cooperative.
