i tested the patch and it just kind of does what it says. im super grateful for it but its really only for people who would be bothered by this sort of thing (wrong title screen) in the first place

Lovely initiative! Is this basically the Japanese version but with added official English translation?

The reason why the name was changed is because of the Eternia name associated with He-Man (the Eternia name was owned by Mattel), and to make it as a sequel to with Tales of Destiny in the West since Tales of Phantasia wasn't localized until the bad GBA port (despite the real sequel came out on PS2 years later, only in Japan). So the patch reverts it, but also restores the OP theme as well.
