Been playing horror games all my life since I was a kid, so it's hard to get spooked. But Silent Hill 3 is legit scary, that sound design is insane, dark room, headphones, still spooks me even though I've finished it many times already.

Fatal Frame 3 is really creepy too.
 
The only game that ever gave me a scare was the original Resident Evil. For context, I bought it new on release here in the US. The part inside the mansion when you are running down the hall and the zombie dogs burst through the glass the first time you re-enter that L shaped hallway and wasn't expecting it. It gave me a quick jump scare. I paused the game and said aloud "Ok game, you got me"
 
Fatal Frame 2. I remember there's this one jumpscare where the ghost of a little boy quickly shows up in a window if you got close and used your camera next to it. That spooked me real good.
Seconding this. I watched my sister play it when I was very young, and from that point onward I slept with a night light lol.
Even nowadays it still gets me good; probably the best of the Fatal Frame series, if you ask me, though I remember the games on PS2 being pretty solid in general.
 
The Silent Hill games always manage to scare me😱

I know Half-Life 2 is not a horror game, but to a little nervous kid me it might as very well been. Ravenholm scared the shit out of me every time, doesn't help that the rest of the game feels so lonely and hopeless, with the post apocalyptic setting and all that.
I have to mention that I wasn't the one playing, my dad was, but I liked to see him play EXCEPT Ravenhorn, I used to beg him to skip this chapter when I was sitting besides him looking at the screen. There was a particular glitch that sometimes happened to his copy of the game were the zombies whould spawn but stay frozen in place, I almost prayed that the glitch activated every time beacuse it made the experience much more bearable, it also stopped the noises from the zombies so it was a huge plus. I remember dad telling me that "all of those noises are just made by a bunch of guys grunting and having fun in a recording booth, there's nothing to be afraid of", and years later when I heard the reversed message I felt vindicated lol. Still a bit spooky to me even nowadays...
 
Megaman X4, i came expecting another dark fun colorful Megaman Game, i felt in love with Zero since i actually played x5 first, so i picked him, just to be recieved with this shit

Same, gave me a good spook as a kid, same with Nemesis jumping through the window in the RPD Station in RE3 😱
 
Gotta say, playing the Resident Evil 2 Remake with headphones on was a good-bad idea. I'd definitely jumped and yelped a few times. Even with a quick scare or two aside, the game had me hesitant to advance forward a few times. First time encountering Lickers, ooh man, that was tense. Same with the damned sewers...

On the opposite side of the eerie spectrum, from jumpscares to nervousness? Traversing The Glow in the original Fallout was nerve-wracking as can be. Rads are tracked almost invisibly IIRC, and the whole place was eerie as hell. There's supposed to be cool stuff down in that irradiated pit but I just wanted to find the mission objective and GFTO as bad as I could. And if you ask me, the Robobrains down there were way creepier than ones in the 'Thesda games. Especially with the noodly robot arms wielding friggin' shotguns.

I swear, the original Fallout was partly a horror game in my eyes. Just like the original Terminator may as well be a slasher flick. Admittedly, I never got far into the endgame, but some day I'll be properly horrified by it too.

Also I hated the living hell outta the Inverted Catacombs' music in Castlevania: Symphony of the Night. I just wanted to get in and get out as fast as possible, superboss be damned. Oh yeah, and the music in Fallout 1's Necropolis area also made me not wanna be there. Can you tell the music and/or ambience in games matters a whole lot to me?
 
When you put it that way, Fallout series as a whole is pretty damn spooky yeah, not just merely satire comedy. Whenever I went to the sewers in 3 and got ambushed by groups of glowing ghouls my heart jumped out pretty fast, doesn't help that usually at those areas there's no BGM playing unless you turn on the radio lol.
 
My original run of Eternal Darkness (I had no idea what the game was about) fucked with my shit something hilariously.
I don't think something like that can be done again and have the same affect as it does the first time one plays. Especially going in blind.
An absolute trip.

I'll add Silent Hill 2 as a close second because that shit is top-tier eerie.

wasn't too deep into horror games back then so it was early days exploration of the genre for me.
anyways, the apartments creeped me out so bad, i just simply couldn't push through further, i didn't like the darkness, narrow corridors, weird sounds. The worst was the top floor, with the heavy breathing down the hallways.
I get chu with this ,dude.
 
The "game over" screens from Shadow of the Colossus are super unsettling. Especially 10.

theres also a haunted hotel section in Vampire: The masquerade - Bloodlines that really spooked me too

I LOVED THIS LEVEL SO MUCH. It freaked me out so much the first time. The way the it messes with you, it’s peak spooky. Honestly, it’s one of my favorite levels in any game.
 
Resident Evil 3 was the first game to actually scare me, and it did so from the very moment the intro started playing. I don't even know what they did, but they just nailed something with the atmosphere that made me not wanna play it alone at all.
