I'll just say "Outer Wilds". For me no creature, no monster, no situation was that as scary as what happens in Outer Wilds and its core game over loop. It's meaningfully scary for me when you deal with cosmic celestial objects, man. How to run away when all fabric of spacetime collapses on you lol. The game plays with your sense of astrophobia by going beyond of basic fear you feel from what you saw and all.
 
While technically not a game over, the bad ending of the arcade game Spinal Breakers is pretty much one of the scariest things you'll ever come across to the point that you probably won't be able to get any sleep after seeing it. Due to the severe intensity of it, I'll only post a link (click at your own risk) while also explaining on what had happened:

Never played that, but that's creepy af. Guess I still gotta go deep the game over rabbit hole
 
Also, not exactly scary but made me tear up a bit when I got it. The music adds up really nicely, and the total darkness on the credits is too eerie.

 
Gothic was always intriguing and terrifying to me because it has NO GAME OVER SCREEN at ALL.
The game tries to be as immersive as it can be which means that whenever you die, you're just left starring at your main characters corpse, lifeless and still, forever (until you reload your save file at least)
