I got this pic ages ago from some meme site and that's all I thought it was, a meme. Well, it turns out it's a real panel from a manga. Took me a while to find it. It's not the most depraved thing I've ever seen but it's questionable to say the least. If you're really curious then all I'll say is that the name you're looking for is Totsugeki Tonari no Onii-chan.
