Pilot is the basically a prototype for a series, be anime, live action or cartoons, while some succeed, some aren't so lucky, showcase yours
While using the Cartoon preflix, anime and Live action applies

Mine must go to this one, especially in a age that ensued the only thing that changed was the shape of the phones

Edit- Oh, and while this show never even had a pilot episode, and is currently in limbo with no hope to ever being made, I'm sad that the Final Fantasy 9 anime never came to be. It was going to be called Final Fantasy IX: The Black Mages' Legacy, which featured Vivi's children growing up and saving the world. I hate Square Enix sometimes.

Kind of cheating here since this is only a teaser technically, but man I would've loved to see something like this when I was a kid:
This is a project Daisuke Nakayama, a Japanese artist/animator/director, has had floating around for almost 20 years now. I have no idea what has been preventing this from getting off the ground after all this time, but he does appear to still have interest in the project and still draws characters from it so hopefully my man can finally have his day and get this thing out there in some way.
 
Popful Mail. There was a promotional video done as a pitch to studios, but failed to gather interest. It's a shame, really.
 
Don't know if this counts but I wish the BBC dubbed the entire Urusei Yatsura series instead of just episodes 1 & 3.
 
Mockingbird Lane
It was aired once in 2012 as a Halloween Special and was full-on reimagining of The Munsters. Overall kind of had a vibe not too dissimilar to "Pushing Daises" and "Dead Like Me" and I really do wish it had been picked up. I mean, come on! It had Eddie Izzard as Grandpa!
 
I'm not really sure if this counts, but I'm still kinda miffed to this day that ARMS only ever had a prologue comic made that teased a run that never ended up happening.
