It's Mr. Balls for the Mega Drive/Genesis, it's a puzzle game made infamous for being included in AtGames' plug-n-play systems... I kinda like it, I used to play this mini game a lot on Chinese handhelds back in the early days.

If anyone remember the tiger electronics hand held game Lights Out the this is essentially the same game.
 
Update: in the end Devworks was a brazilian studio and this game was first released as Bil Bolha. Tectoy licensed it to AtGames, that translated and released it later as Mr. Balls. :p
 
Everyone is commenting on the title, but I can't believe that box art is real lol... It looks like it was made in MS Paint.

Not sure the term "unofficial" applies. Tectoy was authorized by Sega, their consoles are official Sega products, not unlicensed clones like most NES-based ones.
