I was happy to see some of my favorites (Baroque, SMT, Dark Cloud) mentioned but THIS probably has to be the one I was most looking to see if had been suggested. If OP doesn't mind that it's a PC release and not console.
 
I think the GOG version of VtM: Bloodlines might be pre-patched to work out of the box? Don't hold me to that but I think it was one of their "good old games" series that they were setting up to run that way

Xenogears gets weirder as it goes, for sure. If you're ready to get a little galaxy-brained, and you enjoy giant robots and kung fu, it's hard to go wrong here.

Live A Live has multiple scenarios, each with a wildly different genre and setting. Very fun, easy to get into.

Since you're looking at Fallout, maybe take a gander at Shadowrun? It's not D&D, but a contemporary tabletop RPG with multiple game adaptations. It's basically cyberpunk, but with a kitchen sink fantasy layer thrown on top. There's an SNES game that was interesting, and some modern PC games from about a decade ago that are very easy to run on most any rig.

(Seriously though, Fallout 1 and 2 are phenomenal, try those out for sure.)
ive actually already played the hd2d remake of Live A eviL, its what wet my appetite for more unique settings in rpgs, I also a couple days ago at time of posting just finished the first xenoblade chronicles, i liked the story a lot but the gameplay left a lot to be desired, is the orignal xenogears more traditional in its gameplay (turnbased or atb instead of that weird cooldown xbc1 had)
 
Pandora's Tower for the Wii might be up your alley.

Super Robot Wars Series. OG on nds are JRPGS instead of SRPGs the entire series is best known for, but you can play them too, some are english.

Anachronox. Basically a sci-fi JRPG made by people who worked classic PC FPS games. Tone is somewhere between Blade Runner and Hitchhiker's Guide, very good story and world and can be dramatic at times but never takes itself too seriously.

Arc the Lad Twilight of the Spirits

It might sound strange because the later elder scrolls games moved towards more generic medieval fantasy settings, but Morrowind is one of the first games that comes to mind when I think of an rpg with an alien setting

The original Xbox 360/PC release of Mass Effect

Magical Starsign is sci-fi fantasy but not Star Wars like

Evolution 1 & 2 are Dreamcast games that were later ported to the Gamecube (similarly to Skies of Arcadia & the Sonic Adventure titles) as Evolution Worlds. The Wiki for the re-release is https://en.wikipedia.org/wiki/Evolution_Worlds. I recommend against the Gamecube release as it radically scaled back the first game for disc space issues and censorship.
