This sort of does not need a translation. I streamed this in full years ago and it is 50% japanese and 50% english. You can pick both the japanese Mosquito from 1 or the new American one and they will go around sucking both families. The plot for this game is that the family from 1 goes to Hawaii and the mosquito hops in their luggage. Since the American family is American, they are already in english and you know what you need to do. Unlike the first game however, the mosquitos here have a job to do and this game doubles as an acupuncture simulator where sucking the right point will relieve the ailments of the people you suck so they will say things like "I have a back ache" and there is a guide in game included already in english that tells you about each point. However you still cannot understand the japanese family since they are speaking japanese and you just have to guess. I can easily recommend this game for how unique and wild it is though as it is arguably crazier than the first game. Hope this helps in some way if you wanna play a couple levels since the american mosquito starts out with 3 or so levels you can play without knowing japanese
