NBA Jam on The PS1 had it's difficulty the wrong way round!
Outrun on the ZX Spectrum has the had written in reverse!
Kang Fu on the Amiga CD32 wouldn't boot because the BIOS took up too much RAM
Shadow of the Beast on the Genesis wasn't adjusted properly from it's PAL release and ran too fast
The Death and Return of Superman on Mega Drive crashed on the 2nd level!
