A funny one for me, but it would have to be the first encounter with a regenrator in RE4. As a guy who was used to be hilariously irrationally afraid of zombies when i was younger, RE4 happened to be the first horror game I really got a decent ways in and was on track to beat. That sequence however, had me so shook up that I ended up turning the game off and deleting my RE4 save data off my PS2 right after (its okay I ended up going back to it months later and conquering my fear).
 
I would get really scared watching my uncle play Mario Party 3 and landing on the Bowser tile. For some reason I thought the Bowser model seemed SO SCARY. I would run away after just looking at it. :((
 
For me it was in Eternal Darkness. I was playing as a character and my sanity meter was low, when the game froze for a sec and the intro movie played. I thought I had somehow reset the game and lost all of my progress.
 
I remember a few moments in Alone in the dark: The new nightmare.

The appearances of characters are mysterious and frightening to say the least, specially the fking Joshep Edenshaw, the witch doctor.
 
I remember these in the order they happened.

1. Going back up the stairs near the elevator in Resident Evil 1 Remake, a zombie came from the corner while I was going up. Did not expect it back then, lost at least a year off my life from that scare.

2. Original RE4, the military Ganado that bursts out of the fridge on fire. I'm pretty sure I threw up my heart that day.

I've ingested so much horror content since then so I don't really get spooked but games like Alien: Isolation, P.T. and so on have gotten the better of me.
 
Brookhaven hospital in Silent Hill 2.
This year I did a Marathon of the first four Silent Hill and when I played 2 I did it with the definitive edition mod that makes everything 4K. I was very confidant. After all I completed 1 not too long ago and my playtrough on 2 was going very fine. Then Brookhaven came and introduced the nurses with pistols that can kill you very quickly. At that moment I was scared as hell to enter any room and also the light went off that night so yeah I almost shit myself ?

Resident Evil 2 - I rented it for the N64 and I was not ready for this random ass jump scare. It got me so good that it made me fall back over my chair ? I think part of the reason was because it happens so up close to the screen. The interrogation room was another one that got me good.

Those one sobs in Dead Space that you can't fully kill and probably the boys of silence jumpscare in Bioshock Infinite.
 
Definitely booting up Resident Evil for the first time at a friends house at 3 AM. Very first thing I did was try the front door of the mansion, and the resulting shriek out of me woke *everyone*.
 
I played The Suffering, and was walking along a very dark prison corridor, the batteries in my flashlight were almost dead, and a monster jumped in front of me with a loud screech. I played in the dark room with headphones on and it scared the crap out of me.
Quite a picturing skill!
I think at that moment it was you who was suffering!

Dropped my PS1 Memory Card into a filled bathtub when I was a kid. The thought of losing my save files were the most terrifying thing back then. The card wasn't broken, tho, so the saves were safe.

2nd most terrifying thing is playing PT for the first time. So, you know how terrifying the first one was for me lol. The experience got me very careful with everything. I keep all my cards and USBs in a waterproofed pouch
 
most terrifying.... when you spend your saving for months for gacha, and all you got are useless stuff.

in game, would probably when play slenderman, its my first horror game
 
Honestly the metal gear solid series spooked me a couple of times (despite not being horror games) like: the first encounter with gray fox in mgs1 , the entire last part of the story of mgs2 , that one river part , a lot of instances in mgsv the game honestly felt like horrific fever dream, the only horror game I have played was silent hill 1 and to be honest there wasn't anything really scary in it aside from that one fucking cat being the scariest part of the game
 
Fatal Frame 1 when I was a kid, I genuinely screamed when I saw the first ghost.
Nemesis in RE3 and Dahaka from POP WW were nightmares to me.
 
Years ago, I was playing Return To Castle Wolfenstein game in house of a relative, and I had played it a few time before too, in house of same relative, (e.g. every year or so)... and i played it a few times later too...
But that one time, it was so strange! I was so scared that couldn't progress in game! despite exactly knowing what was coming!
The ghosts suddenly had become so scary for me! and in that level where half-bodied robots were attacking their creators, i was almost shaking to get near them!
It never had happened before, and never happened after that either!
 
when i was a kid i didn't know in re1 you could actually open the front door to the mansion. i was at my baby sitter's house overnight and i had the lights off, and aw man; that shit haunted me
for a WEEK
 
Resident Evil 1 is probably the first time I was terrified by a game.

My most terrifying moment in gaming probably had to do with being super close to death when I hadn't saved in a long time and being close to losing hours of progress. I know its happened a few times, but I can't point out a specific example.
 
Alien 3 for the Amiga. For those who haven't played it - it's an ok-ish maze platformer in which your goal is to find and rescue all hostages and then reach the exit while avoiding xenomorphs (you can shoot them but ammo is severely limited). You also have to be quick since there is a time limit like in most games of the time.

So, what happens if you fail? You get to see every hostage you failed to save die one by one as aliens burst out of their chests in a rather gory fashion.

4-year-old me suffered a few restless nights after seeing that for the first time.
 
There was one precise moment in RE7 that i don't think i could replicate again.

I was hiding without moving on the second floor, once i stopped hearing footsteps i decide to come out to advance, only as soon as i go to the corner of the hallway, Jack instantly jumps out with an extremely amused tone saying "where you going?", as if he knew exactly what i was doing and he was toying with me, that mere idea sunk me in fear before i bolted to the other side.

I played a lot of survival horror games, but that mere idea still lives rent free in my head.

As a runner-up, i say lots of the sanity effects of Eternal Darkness can really fuck with you mentally, even if most of them are tailored made for SD TVs these days.

the brightness went down a lot, youre in your tiny, feeble water craft... You start hearing a loud bellowing noise but dont know from direction it comes from
