So I have a whole heap of Mangas which aren't well-known, like at all, here are a couple:

Green Blood (maybe not as hidden, but still not talked about):

A-bout (Now this one is truly a hidden gem and I don't know why, this is a really good Highschool action/comedy Manga, highly recommend):

It's criminal how so little people talk about Molester Man. It's one of the most wholesome and funny things i ever read.

Also, Onani Master Kurosawa got me by the gut and threw me in the trash, i felt all emotions at the same time, but i don't know if it's exactly a hidden gem

It's about a Nekomata who is rejected by Jokai for looking too much like a human and for humans for looking too much like a jokai, so he goes wandering between villages, and in each village takes place is a chapter.

Not sure if it's really a hidden gem, but it was good for me. It's a horror, comedy manga, Mieruko Chan

Kakarate Shoukoushi and A-Bout are both excellent and could easily be popular if better known.
If you like weird humor and surrealism with your action/violence, Bakune Young is a classic. The same author made Castle of Dragons which is also pretty crazy. QP is one of my favorite manga of all time, it's by the same author as Crows and Worst but it was seinen instead of shounen so it's little more mature and "real" and it's shorter (I think 8 volumes) so it feels like a story that was fully planned and told from the start rather than a long series where some stronger guy always comes along to keep it going. IMO it's the best delinquent manga.

Tokyo Kettou Kanjou-sen is about a fighting tournament. Story takes a backseat as characters and action really shine, everyone has a different weapon and fighting style. Kengan Ashura's weirder and weapon-based cousin. Really hype action manga and still ongoing.
 
If you like disturbing and weird manga this one is for you.

I'd also recommend...

Bokurano: First, I'll warn people... it has some content that some people might be really sensitive to, and it's incredibly dark... but it's fantastic with a great emotional payoff.

Family Rivalry Killing Battle: Another pretty dark one, but I'd say a lot less... raw? It's a death games series about different families in a town competing against each other. I can get kind of weird.

Dorohedoro: Probably the most well known out of my recommendations, by no means unheard of... but it definitely is underviewed and I don't hear people talk about it a lot. It's very odd and surreal, and the anime adaptation is only a small portion of the plot and not particularly great. I'd say there's a bit of an "accessibility" barrier here, but it's totally worth it.

Various Junji Ito work: Yes, he's super well known... but a lot of his work isn't. Everyone knows Uzumaki, Gyo, Tomie, Long Dream, The Hanging Balloons, etc... but so many great stories go unread by even fans of his. Some I'd specifically recommend...
