Why is there such a file size difference between (Japan) and (Brazil, Japan, USA, Europe) version ? (133.06 KB vs 20.42 KB)

The Japanese version came on a 256KB cartridge while the rest of the world got a 128KB cartridge so as a result half the game was cut back in the international versions.

Japanese game has more courses, 10 vs just 5 on international release

Because the japanese version is bigger than the others.

https://tcrf.net/Enduro_Racer_(Sega_Master_System) The Japanese version had more levels (and the one on first screenshot), but due to the costs of the 256kb chips the western versions used cheap 128kb chips and just repeat the 5 levels instead.
