The file size is due to the music files on mp3. The actual game is 8MB.
To play this on Retroarch you have to use the core included, a modified genesis plus core, else the game will play a fake low res "mockup".

There are better cores now. Don't use the one included with the game.
- For Windows download paprium_genplusgx_expanded_f8fe365_win64.zip
- For Android download paprium-genesis_plus_gx_libretro_android_all.7z and choose from there.
- For Linux download genesis_plus_gx_paprium_libretro (Linux x64) (a0b5a03).7z
So go to your Retroarch cores folder, extract the corresponding core depending on your system (the .dll if windows and .so if Android/Linux).
After that just load content and find the game .bin.
